I am married; my wife’s name is Emily Osbern. My places of residence since the war have been: from about 1865 to about 1879, in Clay County, State of Kentucky, My P.O. address being Big Creek. The following is a full and complete statement of all the service ever rendered by me in the military or naval service of the United States: I enlisted at Boonville, Kentucky, on the 24th day of December 1862, to serve one year in Company L of the 14th Regiment of Kentucky Volunteers and was honorably discharged there from at Camp Nelson on the 24th day of March 1864 I now have the following injuries, diseases, or disabilities, each of which was caused or incurred as stated opposite thereto and neither of which is the result of vicious habits on my part: Name of disease what part of body what caused it? Well known to me to be reputable and entitled to credit, and who, being duly sworn, declares in relation to aforesaid case as follows: I was a member and belonged to the same Regiment of claimant James Osborn on or about the 15th day of September 1863 while we were on a scout in Perry County Kentucky the claimant James Osborn was taken sick with something like white swelling in the left ankle and leg. His leg became very much swollen and looked red and inflamed. And I have known the above named facts for the reason as follows: I was his brother comrade in arms, also belonged to the same mess, eat and slept with him and have lived a near neighbor to him ever since and before the war. On this 26th day of July A.D., 1887, personally appeared before me, a County Court Clerk in and for the aforesaid County and State, duly authorized to administer oaths, IRVIN JONES, aged 50 years, a resident of Bull Creek, in the County of Leslie and State of Kentucky, whose Post Office address is Hyden Leslie County Kentucky. Well known to me to be reputable and entitled to credit and who being duly sworn, declare in relation to the aforesaid case as follows: I was a member of the same Company (L) 14th Regiment Kentucky Volunteer Cavalry with the claimant, on or about 15th of September 1863 while on a scout in Perry County, Kentucky the claimant James Osborn was taken with something like White Swelling in his left ankle and leg. His leg became very much inflamed and swollen and I have known him ever since that time and he still complains of his leg hurting him And as above stated I was in the same Company and I belonged to the same mess that claimant did and from this and I being a near neighbor to claimant I know the above facts as stated. Eversole and was honorably discharged at Camp Nelson, Kentucky on the 24th day of March, 1864; that his personal description is as follows: age 46 years; height 5 feet 10 inches; complexion, fair, hair, light; eyes, grey. That while a member of the organization aforesaid, in the service and in the line of duty at Irvin, in the State of Kentucky on or about the 5th day of March, 1863, he was taken with a severe pain in my left leg supposed to be white swelling. My leg became sore and swollen up. I could not walk on it and my Regimental Dr. pronounced it the (disease) white swelling. I was also affected with kidney disease from which I am in daily pain. That he was treated in hospitals as follows: at Irvine, Kentucky I was treated in hospital about March 5th, 1863 That he has not been employed in the military or naval service otherwise than stated above. That since leaving the service this applicant has resided in the County of Leslie in the State of Kentucky, and his occupation has been that of a farmer.
