a. Statement by Sam:

was a member of Co. D, 47th Ky regiment. Born April 1844 at Bishop Bend . Ever since I was discharged from the servis and my occupation had been farming. Dr James Fletcher has doctored me since the war and the said Dr Fletcher is died on Sept 1887, I had the fever in the year 1866 in august about the 15th. I did not have any Pherium? for the fever. i have worked all I was able to since the war at farming . On a yearly average prevented from following my occupation 1/2 the time. I have been confined to the house or the bed about 1/4 of the time yearly.

b.Surgeons Certificate: NM Scales, TP Caldwell, HS Pitman, Feb. 8 1888; LondonKy. Pension Claim # 624338.

c.Married Jan 12, 1866 to Mary Jane Howard.

d. Medical : Height 5'8";, Blue Eyes, Light Hair, 160 lb., light complexion. Sam died age 74 in 1917 of chronic brochitis and heart & lungs.

d. Request for Widows pension: A letter written herself it seems. Jan 10, 1917. My husband Samuel Bishop died June 7, 1917... he was drawing a pension under certificate 3 473650. I married him a short time after the war. Please send me a proper blank.. I am 73 years old. Mary Bishop.

e.Widows Claim by wife Mary Jane age 76 in 1917.

f.Sam enlisted at Booneville Aug 10, 1863 and was discharged Dec. 26, 1864 at Lexington.
