                 DECLARATION OF INVALID PENSION

State of Kentucky county of Caly
On this First day of September, 1890 Personally appeared before me
Within and for the aforesaid Alexander Bishop aged 47 years, a
Resident of Teges county of Clay state of Ky. Who being duly sworn
According to law, declares that he is the identical Alexander Bishop
Who was enrolled on the 10th day of Oct 1862 in Co. I 14 Ky Vol Cav.
In the war of the rebellion and served at least 90 days and was
Honerably discharged at Camp Nelson Ky on the 14th day of Feby 1864
That he is ???????? unable to earn support by reason of injury to
Left ankle, disease of right side, name unknown.

That said disabilities are not due to his vicious habits, and are
To the best of his knowledge and belief permanent.  That he has never
Applied for pension.  That his post office address is Teges County
Of Clay state of Ky.

Also personally appeared Jesse Allen, residing at Teges Clay co Ky.
And Wm. Abner, residing at Burning Springs Clay Co Ky.  Persons whom
I certify to be respectable and entitled to credit, and who being by
Me duly sworn, say they were present and saw Alexander Bishop, the
Claimant sign his name to the foregoing declaration; that they have
Every reason to believe, from the appearance of said claimant and
Their aquaintance with him 25 years and 10 years respectively that 
He is the identical person he represents himself to be; and that
They have no interest in the prosecution of this claim.

In the matter of the org pension claim no 928,844 of Alexander Bishop
Late of Co. I 14th Regt. Ky. Volo. Cavl.
Personally appeared before me a County Court Clerk in and for the 
Aforesaid county and state. The claimant Alexander Bishop who being
Sworn by me states that he claims ???????? injury of left ankle and
By which was broken and mashed up by a large slide loaded with ?????
Turning over on him which renders his leg stiff and nearly useless
Contracted near his house about the 19th day of Nov. 1886 and claims
For disease of the side which comes on him gradualy since the war
Further states that he has not been in the military or naval service
Since Feb the 14th 1864

Sworn and subscribed by me by said affidavit. I certify that this
Affidavit was fully read to him before his signature execution
And that I have no interest in this claim for pension.

On pension claim NO 692,863 of Alexander Bishop Co I 14th Ky Cav
Personally came before me a notary public in and for the aforesaid
County and state, H.G. Sparks M.D. whose post office address is
Oneida county of Clay and state of Kentucky.  Well known to me to
Be reputable and entitled to credit and who being duly sworn 
Declares in relation to the aforesaid case as follows.
That he is a practicing Physician and has been aquanted with the
Above named soldier for about 15 years and has this day carefully
Examined him find him suffering from rhumatism of the right side
With tenderness over the lower border of ribs and sourness of
Mussels of back and neck also injure of left ankle joint with
Stiffness of said joint and soreness, fingers on right hand closed
And partial ???????.  Claimant also has piles with inflamation and
Hemorhoids.  In my opinion claimant is at least 3/4 disabled from the
Preformance of manual labor by reason of said disability and that I
Have no interest direct or indirect of the prosecution of this claim.

DECLARATION OF PENSION


State of Kentucky County of Clay on this 20th day of April A.D. one
thousand nine hundredand seven Personally appeared before me a Justice
of the peace within and for the county and state aforesaid, Alexander
Bishop, who being duly sworn according to law, declares he is 64 years
Of age, and a resident of Teges county of Clay state of Kentucky, and
That he is the identicalperson who was enrolled at Boonville, Owsley
County under the name Alexander Bishop on the 10th day of October 1862
As a private in Company I, 14th Kentucky Cavalry in the service of the
United States in the Civil War, and was honorably discharged at Camp
Nelson, on the 24th day of March 1864.

That he was not employed in the military or naval service of the United
States otherwise than as stated above.  That his personal description at
Enlistment was as follows:  Color of Eyes, BLUE; Color of hair: BLACK;
That his occupation was a farmer; that he was born the 22nd day of 
August 1842 at Teges Kentucky.

That he makes this declaration for the purpose of being placed on the
Pension roll of the United States under the previsions of the act of
February 6, 1907.

That his post office address is Teges, county of Clay state of Kentucky

Resident of Teges Newfound county of Clay state of Kentucky, and that
He is the identical person who was enrolled at Booneville Ky. Under
The name of Alexander Bishop on the 10th day of October 1862 as a
Private in co. I 14th Ky Vol Cavl in the service of the Civil War
And was honorably discharged at Camp Nelson on the 27th day of March
1864.

That he was not employed in the military or naval service of the
United States otherwise than as stated above.  That his personal
Description at enlistment was as follows:  color of eyes; grey
Color of hair; black that his occupation was a farmer that he was
Born Aug 22nd 1842.

That his several places of residents since leaving the service
Have been as follows:  Newfound Creek, Clay Co. Ky.
That he is a pensioner under certificate # 692,863
That his post office address is Newfound Clay Co. Ky.

DECLARATION OF PENSION
ACT OF MAY 1920

State of Kentucky county of Clay
On the 24th day of may 1923 personally appeared before me a notary
Public with in and for the county and state aforesaid, Alexander
Bishop, who being duly sworn according to law, declares he is 80
Years of age and a resident of Newfound county of Clay state of
Kentucky and that he is the identical person who was enrolled at
Booneville Owsley County Ky., under the name of Alexander Bishop
On the 10th day of October 1862 as a private in Co. I 14th Kentucky
Cavalry in the service of the United States in the Civil War and
Was honorably discharged at Camp Nelson Ky on the 24th day of March
1864 that he also served none other. That his personal description
at enrollment was as follows:  color of eyes________  color of
hair _________  height _______ complexion _______  and that his
occupation was: Farmering that he was born August 22nd 1842 at
Newfound Creek Clay Co Ky.  That he requires the regular personal
Aid and attendance of another person on account of the following
Disabilities:  By reason of age and physical disabilities as
Fo;;ows: rheumatism and ???? Kidney and bladder trouble. Trouble
More or less each day spell of blindness and ????? can not stand
On feet, falls
That since leaving the service he has resided at Newfound in Clay
County Kentucky and his occupation has been farming.  That he has
Applied for pension under original No 692863, that he is a pensioner
Under certificate 692,863.  That he makes this declaration for the
Purpose of being placed on the pension roll of the United States
Under the provisions of the act of May 1, 1920.

Aforesaid.  John Dean who being duly sworn according to law, declares
That he is 26 years of age and a resident of Newfound Clay County,
Kentucky and that he is personally and well acquainted with the claimant
Alexander Bishop and that he renders his aid and attendance daily to
The said claimant in the way of helping the claimant to arise to his
Feet and conducting him to his seat immeadiatley after falling from some
Kind of helpless spells he takes.  The claimant is not confined to the
House at all times nor to his bed, only a portion of the time, and
Further states he is not related to the claimant.

  In the Matter of Alexander Bishop whose post office is Newfound
Clay Co. Kentucky.
This is to certify that I am a regular practicing Physician and
Have been his family physician ever since I began practicing.  Upon
Examination today I found that he is suffering from kidney trouble
And fro???  Rhumatism??  Bowl arbeular??  And muscular because of
The kidney trouble.  He has blindness at times and has falling attacks
He says when he has these attacks he must have the aid and assistance
Of another person.  By reason of his age and kidney trouble along with
The rhumatism?? He suffers from he is totally disabled to do any kind
Of work and is not able to care his own support.
