  It is alleged that the above-named man enlisted Aug 17, 1863, and served as a Pvt. in Co. A, 52 Regt Ky Mtd Inf, and was discharged at Bowling Green, Ky on Jny 18, 1865.
   
  Chesterfield, Macoupin Co, Ill, Jan. 8, 1892, in regard to Pension Claim of HAMILTON D. WADE, Private Co A. 52 regt ky Vol Mtd Inft. Claim No. 1030 440
  This day the above claiment HAMILTON D. WADE who beling personly known to me, apper before me, JOHN O. ABBATT a police Magistrate in and for the county and state above writen and on oth says that sometime in the Spring of 1883, while working on a Building with JOHN J. LEACH, that thay both fell from said Building and thet he the siad HAMILTON D. WADE receivd the injury to his wrist and that it was not dur to any vicious habits of his, and that he beleaves it is a permanint injur. in witness whereof he hereunt sets his hand seal, this eighth day of Jan, 1892.   In the Pension claim #8467212 of HAMILTON D. WADE late of Co "A" 52 Reg't Ky Vol Inft I will say my age is Sixty seven years. I have been a practising physician for fifty four years my residance is Chesterfield Illinois.
  I am well acquainted with HAMILTON D. WADE the applicant for the last fourteen years.
  That in May 1883 he fell from a building and injured his left arm. The ulna was broken above the wrist and the lower head of the same was dislocated ruptereing the ligaments of the wrist, so that he has been unable to do any kind of work that requires the full use of both hands and arms. I further say that I have no interest in the prosicution of this claim. C.H. ???????? MD

  In the Pension claim #846722 of HAMILTON D. WADE late of Co "A" 52d Reg't Ky Vol Inft. I will say. My age is 52 years and residence is Chesterfield Ill.
  I am well acquainted with the applicant for the last 14 years.
  That sometime in year 183, the applicant was laboring for me, shingleing on a building when the scafoding gave way and he fell to the ground and injured his left arm. and it has never gotten sound and well and I would say that he is incapassatated from earning a liveirng from said cause two third or at least one half. Further will say I have no interst in the prosicution of this claim. JOHN J. LEACH

I am well acquainted with the applicant and have been for about 14 years. I know that he has a bad lame arm and that he is incapassetaded from earning a supprot two thirds

  Sir; Referring to you above certified clai for increase of pension under the act of Feb 6, 1907. you are ???? that you shuld furnish the evidence indicated in the attached circular letter. if inable to do so the fact and reason shuld be stated under oath and you shuld state the true twnship conty and state in which you lived in 1850 and 1860 with the full names of your parents if you resided with them or if not of the persuns with whom you lived during those years.

  On this 22nd day of June, A.D. 1910, personally appeard before me a Notary Public, in and for the County of Macoupin and State of Illinois, HAMILTON D. WADE, aged 75 years, well known to me to be the identical party he represents himself to be an dowrthy of credit of the County of Macoupin and State of Illinois, who being duly sworn on his oath, doth make the following statement. That he is the identical person who is making application for pension under certificate numbered 846.722, that there is no public record of his birth, as the laws of the State of Kentucky did not require it at that time. That there is no baptismal record of his birth, and that he is unalbe to furnish a copy of the family record of his birth on account of it's having been lost. That he resided near Scottville, Allen County Kentucky, during the years 1850 to 1850 inclusive with his parents JOHN WILLIAM WADE and ANNIE WADE, that he was married in 1859 and resided in 1860 on the same farm on which he was raised.   In the matter of HAMILTON D. WADE application for pension, testamony showing ??? - heart disease and disease of stomach from Nov. 6-10 to Feb. 26 -02
  On this 24 day of June, A.D. 1902, personally appeard before me, a notary public in and for the aforesaid County, and State, duly authorized to administer oaths S.A. HUFFMAN, age 28 years, a resident of Chesterfield in the County of Macoupin and State of Illinoins, whose Post-office address is Chesterfield Illinoins and WALTER J. BRANDT, aged 45 years, a resident of Chesterfield in the County of Macoupin and State of Illinois, whose Post-office is same, and who, being duly sworn, declares, each for himself, in relation to aforesaid case as follows: Having been personally acquatined with Mr. WADE for over 3 years, I blieve him to be afflicted with chronic rheumatism heart diseach and disease of stomand and bowels to an extant that makes him physically unable to perform manual labor. S.H. HUFFMAN, M.D.
  Having been personally acquainted with applicant, Mr. WADE, for 2 years, I believe him to be afflicted with rheumatism - heart disease and disease of stomach bowels to that extent that makes him physicially unalbe to perform manula labor. The applicant being a tenant on farm owned by witness - he being with him daily and hearing complaints and knows him to be thus troubled. WALTER J. BRANDT

  On this 17th day of May, A.D. one thousand nine hundred and twelve personally appeared before me, a Notary Public within and for the county and State aforesaid, HAMILTON D. WADE, who, being durly sworn according to law, declares that he is 77 years of age, and a resident of Chesterfield, county of Macoupin, State of Illinoise; and that he is the identical person who was enrolled at Scottsville, Allen County, Ky under the name of HAMILTON D. WADE, on the 17th day of August 1863 as a Private in SAMUEL J. RICHARDs Company of the 52nd Regiment of Kentucky Infantry Volunteers and in the service of the United States, in the Civil war, and was honorably discharged at Bowling Green, Warren Co. Ky. on the 18th day of January, 1865.
  That he was not employed in the military or naval service of the United States otherwise than as stated above.
  That his personal description at enlistment was a follows: Height. 5ft. 11 in; complexion Dark; color of eyes Dark; color of hair Dark; that his occupation was Farmer; that he was born May, 2nd, 1835. in Allen County Kentucky.
  That his several place of residence since leaving the service have been as follows: Bowling Grenn Kentucky, Chesterfield Illinois
  That he is a pensioner under certificate No.846722
  That his post-office address is Chesterfield, county of Macoupin, State of Illinois
