Muster Roll 22 September, 1861, enrolled 3 September, 1861 from Boonville, Kentucky for three years

Muster Roll 10 December, 1861, absent with fever

Muster Roll January 1863, present

Muster Roll 10 April, 1863, absent, sick, on Hospital Steamer from 1 March, 1863

Muster Roll March and April, 1863, absent, sick at Young's Point Hospital, from 2 March, 1863

Army of the United States, Certificate of Disability for Discharge: Lewis Reynolds of Captain Thomas Wilson's Company D of the 7th Kentucky Infantry, volunteers was enlisted by Captain A.L. Clark, camp Dick Robinson, Kentucky on the third day of September, 1861, to serve 3 years; He was born in Owsley County, Kentucky, is twenty one years of age, five feet, eight and 1 1/4 inches high, fair complexion, blue eyes, light hair and by occupation when enlisted a farmer.

said soldier has been unfit for duty for the last seven months by reason of chronic disease and Small Pox, lost his right eye by cause of Small Pox; I certify that I have carefully examined the said Lewis Reynolds of Captain Wilson's Company, and find him incable of performing the duties of a soldier because of chronic Diomhoe and disability resulting from intermittent and also loss of right eye from Small Pox. He is unfit for the Infantry Corps.

Discharged this fourteenth day of September, 1863 at Brashear City, Louisiana.
